If, after the call, Dr. Guthrie and you decide to move forward, we will send you intake forms and some practice documents to sign online. You are also encouraged to upload any medical records, lab work, or other information you would like to share.
3. Day of the Appointment
Have a list of any medications you are taking. You will have a comprehensive evaluation with Dr. Guthrie, discuss your treatment options and next steps, and be on your way to feeling better.
